Position:
        Home Health Aide (HHA)

Function:     
As a direct care staff member, the HHA works closely with the Administrative Assistant or all others on management staff to assist in the coordination of a treatment program for clients which ensures quality programming and timely follow-through.

Qualifications:

1.   High school diploma or equivalent.
2.   Experience working with individuals with traumatic brain injuries, developmental    
      disabilities,or other catastrophic injuries/illnesses or the elderly.
3.   Demonstrates excellent oral communication and written documentation skills.
4.   Demonstrates strong problem solving skills.
5.   CPR certification/first Aid training-preferred but not required.

Objectives:

1.   High school diploma or equivalent.
2.   Experience working with individuals with traumatic brain injuries, developmental disabilities,
      or other catastrophic injuries/illnesses or the elderly.
3.   Demonstrates excellent oral communication and written documentation skills.
4.   Demonstrates strong problem solving skills.
5.   CPR certification/first Aid training-preferred but not required.

Duties:
The HHA is responsible for the day-to-day care of the clients in accordance with established treatment plans.

1.    Provide direct assistance and/or supervision with Personal Care (i.e.bathing, toileting, dressing, meal preparation, feeding, errands, etc) and Homemaking, (i.e. washing dishes, vacuuming, dusting, cleaning kitchen/bathroom, etc.) as required.
2.    Home exercise/ROM program as required.
3.    Accompany client to community outings as required.
4.    Medication cueing as necessary.
5.    Transportation to medical appointment, school, work, or recreational activities as necessary.
6.    Ability to participate in emergency preparedness.

Communication/Documentation:

1.    Verbal communication with management staff of changes in client condition or behavior.
2.    Completion of all required documentation (i.e. progress notes, incident reports, behavior tracking forms, etc.

Other Assignments:
