Client Bill of Rights and Responsibilities
As a client, you have the right to:
- Choose your home care provider.
- Be treated with consideration, respect, and full recognition of your dignity and individuality, including privacy in treatment, personal care and respect for personal property.
- Receive a timely response from Interdependence Home Care regarding your request for home care services. To have access to the provider 24 hours a day, 7 days a week.
- Be informed in advance of the charges of services, including payment for care expected from third parties and any charges you will be expected to pay.
- Have family involved in decision making as appropriate concerning your care, treatment and services, when approved by you or your surrogate decision maker and when allowed by law.
- Receive appropriate and professional quality home care services without discrimination due to your race, color, religion, national origin, sex, age, disability, marital status, and sexual preference.
- Know the name and the qualification of all of the provider’s personnel who provide home care services to you.
- Privacy and confidentiality regarding the services that the provider provides and to review your clinical record upon request. A third party shall not be given a copy without authorization from you except as required by law and third party contracts.
- Participate in the development and periodic revision of the plan of care, and to be informed in advance of any changes to the plan.
- Accept or refuse treatments to the extent permitted by law and to be informed of the consequences of such action.
- Be given reasonable notice regarding the termination of services or the anticipated transfer of your home care services to another provider.
- Formulate advance directives and be informed of the provider’s policy to honor them.
- Voice grievances and suggest changes in service, personnel or lack of respect for property without being subject to discrimination or reprisal.

- Be free from mental, verbal, sexual and physical abuse, neglect and exploitation by the home care provider.
- Be informed that care is evaluated through the provider’s quality assurance program.
As a client, you have the responsibilities to:
- Give accurate and complete health information concerning your past illnesses, allergies and other pertinent information about your health, plus any changes in address, and phone.
- Inform the provider when you will not be able to keep a home care visit.
- Participate in development and updating the plan of care.
- Request further information concerning anything you do not understand.
- Treat the staff with respect and consideration.
- Provide a safe home environment in which your care can be given.
- To inform the provider of any problems or dissatisfaction with services.
